Abstract
- 1.1. An emphysematous lobe seen on x-ray, and no obvious obstructing mechanisms are necessary criteria for the diagnosis of congenital hypertrophic lobar emphysema in infants with respiratory distress and occasionally with cyanosis.
- 2.2. Thirty-four cases purporting to represent this condition under a variety of names have been reviewed. In 19 of these cases, no etiologic mechanism could be found, and these may be classified as congenital hypertrophic lobar emphysema. The remaining 15 cases should be designated congenital obstructive lobar emphysema since some obstructive phenomenon was identified. Future studies may determine whether or not this is a valid separation.
- 3.3. All cases involved upper or middle lobes.
- 4.4. Five cases which meet the criteria for the diagnosis of congenital hypertrophic lobar emphysema are added to the literature. Case histories are appended, and criteria for the diagnosis, both clinical and radiological, are discussed.
- 5.5. A plea is made for increased awareness of this entity since prompt surgical intervention may be lifesaving.
