Abstract
Reports of 17 patients with portal hypertension complicating hepatic sarcoidosis were
collected from a review of the world literature. An additional experience with this
syndrome is reported. None of these 18 patients had hepatic involvement as the sole
expression of sarcoidosis. Severe Ever dysfunction was uncommon; only one patient
died of liver failure. Ascites was present in 22 percent of the patients. Hepatomegaly
was uniformly present; 94 percent had splenomegaly, and 50 percent had hypersplenism.
The mean portal pressure was 352 cm. of water. Esophageal varices were present in
78 percent of the patients, 57 percent of whom bled. Four patients with bleeding esophageal
varices had portasystemic shunts, survived, and did well; the other with bleeding
esophageal varices died.
