Abstract
During a two-year period, September 1969 to September 1971, 460 patients underwent
single aortic valve replacement with the Björk-Shiley tilting-disc prosthesis. Among
them, 52 had repair of the mitral valve, 29 had simultaneous resection of an aneurysm
of the ascending aorta, and another 29 had coronary artery bypass. Ninety-two percent
of the patients were in either functional class III or IV before operation. The hospital
mortality rate was 8.5 percent and was strictly correlated to the patient's preoperative
functional class. Eight patients (1.8 percent) experienced early thromboembolic complications,
resulting in one death. Among the 250 survivors who underwent single aortic valve
replacement, 15 (6 percent) died. Late thromboembolic complications occurred in 11
(5 percent) of 223 survivors followed for a minimum of twelve months. Over-all results
in the group of 273 patients operated upon between 1969 and 1970 show that after a
follow-up period ranging from 12 to 27 months, 71 percent of the patients were in
either functional class I or II, 1 percent were in functional class III, and 14 percent
were dead.
