A comparison of intraoperative tests of completeness of vagal section

      Abstract

      The completeness of vagotomy was studied during operation in 769 patients by means of the Burge, Lee, and Grassi tests and checked three months postoperatively by Hollander's test. The tests were evaluated and their limitations discussed. Use of the Grassi test reduced the incidence of incomplete vagotomies from 10 to two percent. In 98 percent of these patients there was still no acid response in control studies made two years later. On the basis of these results, only total absence of an acid response to Hollander's test should be regarded as the index of completeness of a vagotomy; an acid response even if minimal is, in the author's opinion, attributable to incompleteness of the vagal denervation of the stomach.
