Rectal foreign bodies: Case reports and a comprehensive review of the world's literature

      Abstract

      The surgical management of two patients presenting with incarcerated, apparently self-inserted foreign bodies is reported. The large volume of prior literature on this subject is reviewed, with tabulation of 182 previous cases by type and number of objects recovered and with a discussion of patients' age distribution, history, complications, and prognosis. Management problems addressed include history, differential diagnosis of reported pruitis ani, and handling of suspected assault. The variety of surgical techniques used to remove rectal foreign bodies transanally or after celiotomy is discussed. Vaginal foreign bodies and large bowel injuries due to fist fornication, colorectal instrumentation, pneumatic rupture, foreign body ingestion, impalement, and abdominal trauma are also discussed.

      Resumen

      Reportamos en este trabajo el tratamiento quirúrgico de dos pacientes que se presentaron al hospital con cuerpos extraños rectales encarcelados y aparentamente introducidos por el propio paciente. Se revisa la extensa lista de reportes publicados en la literatura mundial sobre este tema y se hace una tabulación de 182 casos reportados. Estos se clasifican por tipo y número de objetos recuperados y se hace una discusión de la edad de los pacientes, historia, complicaciones y pronóstico. Se revisan los casos que fueron problemas de tratamiento y se incluye la historia, diagnóstico diferencial del prurito anal, y el manejo de una agresión sospechada. Se discute asímismo la gran variedad de técnicas quirúrgicas utlizadas para extirpar los cuerpos rectales extraños ya sea por medio de laparotomía o por vía transanal. Se revisan también los cuerpos extraños vaginales y las graves lesiones de intestino grueso ocasionadas por fornición con el puño, instrumentación colorrectal, ruptura pneumática, ingestión de cuerpos extraños, estacamiento y trauma abdominal.
