        Dr Herb Chen (Birmingham, AL): You have focused on the patients that had surgery, but I am interested in the patients who did not have surgery. What was the reason they did not have surgery? Did they choose not to have surgery because they did not survive? Was there a negative event due to the delay of surgery?
